Responsibilities

• Performs chart preparation and scanning according to HIM Prep/Scan Guidelines.
• Reviews images scanned for accuracy and rescans as needed.
• Indexes documents to the correct patient CSN and EMR location, assigns document type, and verifies bar-coded documents.
• Validates and submits death certificates in accordance with state regulations and coordinates with funeral homes.
• Performs Release of Information processing as needed.
• Maintains productivity and submits weekly reports.
