Medical Records & Documentation Specialist

About Horizon Case Management CORP

At Horizon Case Management, we provide structured, compassionate support for individuals navigating complex injury-related healthcare and legal systems. A critical part of our success lies in how we manage documentation—ensuring that our clinical and billing teams have complete, well-organized, and defensible records.

We are currently seeking a Medical Records & Documentation Specialist to help us maintain the integrity and accuracy of our case files. This role is essential to minimizing billing rejections and strengthening the legal defensibility of our work, especially in cases tied to litigation.

Job Overview:

As a Medical Records & Documentation Specialist, you will be responsible for gathering, categorizing, and maintaining medical and legal documentation across multiple active client files. You’ll work closely with case managers, billing specialists, and external providers to ensure that all required documents are timely, accurate, and accessible.

Key Responsibilities:

Record Collection & Management:

  • Collect, organize, and upload medical records, reports, and other relevant documents into case files
  • Identify missing records and proactively follow up with providers, clients, and legal teams
  • Ensure all documentation is properly categorized, named, and stored according to internal protocols

Compliance & Confidentiality:

  • Verify that all records are compliant with HIPAA and legal documentation standards
  • Maintain the confidentiality and security of client files across all platforms
  • Stay current with best practices in medical documentation and privacy regulations
  • Communicate regularly with case managers, billing teams, and administrative staff to ensure files are up-to-date
  • Track documentation timelines and status for each case
  • Assist in preparing case files for billing submissions, legal proceedings, or clinical review

Ideal Candidate Qualifications:

  • Familiarity with U.S. healthcare systems, medical terminology, and insurance processes
  • Exceptionally detail-oriented and consistent in documentation practices
  • Confident in identifying and addressing gaps in documentation
  • Strong written and verbal communication skills in English
  • Organized, proactive, and comfortable working independently
  • Experienced with cloud-based systems, EHRs, or case management software
  • Understands HIPAA and confidentiality protocols

Work Schedule & Location:

  • Full-time position
  • Monday to Friday, 8AM – 5PM EST
  • Remote/work-from-home setup

Compensation:

  • Hourly pay: Rate starts at $5 per hour up depending on experience
