Job summary

Highlandspring of Ft. Thomas is seeking a Medical Records Coordinator to maintain resident charts at our skilled nursing facility in Ft. Thomas, KY. The role ensures records comply with State and Federal regulations and facility policies.

Responsibilities include audits, release of information, ICD-9 CM coding in HCS/ECS, coordinating transcription, and preserving confidentiality. Competitive wages with benefits are offered.

Qualifications

• Detail oriented with ability to complete tasks on a need-based schedule.
• Knowledge of HIPAA and confidentiality requirements is preferred.
• PointClickCare experience is preferred.
• Microsoft Word and Excel skills are encouraged.

Responsibilities

• Performs routine audits of in-house charts to ensure completeness and accuracy.
• Notifies staff of deficiencies and follows up for complete records.
• Assembles medical records of discharged residents.
• Analyzes discharged charts for completeness and follows up with staff and physicians.
• Processes requests for medical information as directed by administrators and risk managers.
• Maintains Release of Information log and destruction records.
• Coordinates physician dictation with outside transcription.
• Maintains confidentiality of medical records.
• Files loose sheets and ensures off-site storage readiness.
• Assigns ICD-9 CM codes and inputs diagnoses in HCS/ECS systems.
