Job summary

Prism Health CAP Northwest is hiring a Medical Records Coordinator to safeguard PHI, process ROI requests, and manage documentation within the EHR. This on-site role requires knowledge of HIPAA, HITECH, and the Cures Act, with experience in an EHR such as Epic and strong organizational skills.

You will handle ROI processing, record audits, and secure transmission of information, supporting clinicians and patients across Prism Health locations in Portland.

Qualifications

Responsibilities

  • Assist patients with ROIs or authorizations and explain PHI rules.
  • Review ROIs and process within required timeframes.
  • Log and track ROIs and authorizations in the EHR.
  • Manage electronic queues in the EHR for documents to merge, separate, index and commit.
  • Prepare, index, scan and quality-check all documentation received.
  • Audit scanned documents for completeness, indexing, and accuracy.
  • Receive outside patient records and scan into the EHR.
  • Use secure electronic platforms to transmit information to the appropriate department.
  • Provide health record services for medical offices and support areas.
  • Perform file maintenance and resolve missing health records.
  • Review health records to ensure compliance with medical/legal requirements.
  • Assist clinicians with delinquent/incomplete health records.
  • Expedite health records when patients are transferred or for legal/reimbursement needs.
  • Answer calls from patients, providers, and external health partners regarding records.

Prism Health a program apart of CAP Northwest is excited to announce that we are hiring a skilled and team-oriented Medical Records Coordinator. The Medical Records Coordinator at Prism Health is the custodian of patient records and protected health information (PHI) and thus safeguards PHI. The Medical Records Coordinator responds to requests for patient information according to all applicable State and Federal Regulations as well as the Health Insurance Portability and Accountability of 1996 (HIPAA), the Health Information Technology for Economic and Clinical Health Act of 2009 (HITECH), and the 21st Century Cures Act. The Medical Records Coordinator reviews and processes Releases of Information (ROI) as well as patient authorizations and serves as a resource to patients and staff providing guidance and best practice on the release of PHI. The Medical Records Coordinator receives all incoming electronic and paper patient information and processes it within the patient's health record. The Medical Records Coordinator also provides oversight of organizational compliance with the appropriate use of the EHR including chart audit and review.

This is a Full-Time role working at our Prism Health locations, Prism Belmont in SE Portland and Prism Morris in N Portland. The schedule for this role is fully on-site, Monday-Friday 8:30am-5:00pm

Starting pay is $23.17 which is equivalent to just over $48,201. The Medical Records Coordinator’s pay increases with tenure and the top of the pay scale is $27.16. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

Who You Are
  • Basic understanding of HIPAA, the HITECH Act, and the Cures Act.
  • Basic knowledge of Principles of Confidentiality.
  • Working knowledge of PCs and Windows based applications.
  • Experience working with an electronic health (EHR) record platform, preferably Epic.
  • Basic knowledge of office equipment (i.e., including, but not limited to eFax, fax, copy machine, scanner, multi-line phone).
  • Leadership skills and ability to work with people at all levels throughout the organization.
  • Strong organizational skills and ability to work independently and manage priorities in a busy environment with frequent interruptions and time demands
  • Demonstrated courtesy, helpfulness, and respect in dealing with customers internal as well as external.
  • Mature and professional demeanor evident in both written and phone correspondence; must have exceptionally strong oral and written communication skills.
  • Ability to work independently as well as to initiate and successfully conclude projects with no supervision.
  • Demonstrated commitment to advancing equity and inclusion in workplace or community settings.
What You’ll Do
  • Assist patients with the completion of an ROI or authorization, helping the patient understand the rules and regulations around protected health information.
  • Review all ROI’s and process them all within the required timeframe.
  • Use the EHR’s health information module to log and track all ROIs and authorizations.
  • Manage electronic work queues in the EHR for documents to merge/combine, separate, extract, fully index and commit.
  • Accurately prepare, index, scan and conduct quality assessment on all documentation received and scanned in accordance with established procedures.
  • Review and audit scanned documents to ensure all pages in the batch were scanned, pages meet quality criteria with rescanning as necessary, pages are indexed to the correct patient/account, document type, and with the correct description.
  • Receive outside patient records, both in paper and electronic format, and scan documents into the electronic health record and route them appropriately.
  • Use electronic platforms such as fax or email to securely transmit requested information to the proper department, provider, or requestor.
  • Provide all health record services for medical offices/support areas.
  • Perform file maintenance, problem solving missing health records, repair health records as needed.
  • Review health records to ensure that all records contain sufficient information to meet medical/legal requirements, to ensure continuity of patient care, and to support education and research needs.
  • Assist clinicians with completion of delinquent/incomplete health records.
  • Responsible for expediting the completion of the health record when the patient is transferred for care at another medical center, for legal requests, regulatory agencies review, and reimbursement purposes.
  • Answer calls from patients, providers, external health partners, regarding medical records.
  • Perform other duties as requested
