Qualifications

• 5+ years of healthcare experience, including veterinary technician or nurse roles.
• Proficiency with Electronic Medical Record/PIMS systems and high attention to detail.

Responsibilities

• Facilitate new patient referral intake by acquiring, organizing, and summarizing medical records from all providers.
• Build strong working relationships with the Care Navigation Team, including Care Navigator and Hospital Logistics Expert.
• Support the ongoing client journey by ensuring records are accessible and well organized.
