Job summary

Southwest-Womens-Care-P in Chandler, AZ seeks a detail-oriented Medical Records Clerk for a full-time role in the administration office. The position focuses on accurate maintenance of patient records and secure handling of confidential information.

You will coordinate, prioritize, and follow through on tasks with providers, staff, and external partners using the EHR system to file, retrieve, and release records while ensuring compliance with laws.

Qualifications

  • High school diploma or equivalent.
  • Two years medical records experience or equivalent in a medical office setting.

Responsibilities

  • Handle/sort incoming faxes from Provider Flow and route to appropriate staff.
  • Move lab reports, radiology reports, and medical records into patient charts.
  • Prepare and release medical records as requested by patients, facilities, retrieval companies, or attorneys.
  • Release medical records for HEDIS, Risk Adjustment, Pay for Performance reviews; schedule onsite retrievals.
  • Answer incoming telephone calls.
  • Update the Prenatal folder daily.
  • Enter RN messages into patient charts and notify providers.
  • Scan and file records from Banner Desert and other sources.
  • Retrieve hospital records from Banner system as needed.
  • Process referrals and register new patients; verify insurance.

We are looking for a self-motivated individual who is looking for a full- time position to work in our Medical Records department. This position is in the administration office located in Chandle r.

Hours of operation: 7:30am - 5pm Monday - Thursday, 7:30am to 2:30pm Friday
GENERAL JOB DESCRIPTION:

This position serves as a point of contact with patients, providers, attorneys and insurance companies and is responsible for conveying issues regarding the maintenance of medical records. Responsibilities include coordinating, prioritizing and follow through until resolution. All responsibilities are performed according to the established policies, procedures and guidelines within the practice and according to all applicable laws.

QUALIFICATIONS FOR THE JOB:
Education:
  • High school diploma or equivalent
Experience:
  • Two years medical records experience or equivalent in a medical office setting

  • Effective communication/interpersonal skills
  • Perform efficiently with analytical problem solving skills
  • Ability to handle confidential information and sensitive issues

  • Work under minimal supervision and make independent decisions using good judgment
RESPONSIBILITIES:
  • Handle/sort incoming faxes from Provider Flow, move to groups within Provider Flow, email to the appropriate staff or print to offices.
  • Move lab reports, radiology reports, medical records from outside facilities etc., accurately and precisely into the patient charts. Task to the appropriate provider.
  • Prepare and release medicals records as requested. Requests are received from patients, other medical facilities, medical record retrieval companies and attorneys.
  • Prepare and release medical records for HEDIS, Risk Adjustment record review, Pay for Performance review or any other insurance chart review as requested; schedule an onsite chart retrieval appointment if needed.
  • Answer incoming telephone calls.
  • Update the Prenatal folder daily.
  • Put Exclusively RN's telephone messages into the patient charts and task the provider.
  • Sort, scan and task all medical records received from Banner Desert (labs, radiology, ER reports etc.).
  • Retrieve and maintain hospital records from the Banner system.
  • Complete tasks from all staff and the patient portal.
  • Receive referrals for established and new patients. Quick register new patients, forward all referrals for insurance verification.
  • Scan advanced back office scanning from offices.
  • Effectively utilize the EHR system for proper e-filing of documents.
  • Warm transfer of all patient phone calls to different departments.
  • Performs other related work as requested/directed by management.
