Job summary

Stanford Medicine Children's Health is seeking a program outreach specialist to support pediatric providers in private practices and clinics within Alameda County. The role focuses on promoting developmental screening and ensuring appropriate referrals for children and youth.

Qualifications

• Bachelor's degree in a work-related discipline/field.
• Two years of progressively responsible and directly related work experience; experience with child health and/or development and/or pediatric primary care settings is preferred.
• Licensing/certification: None required.

Responsibilities

• Maintains contacts with County Public Health, pediatric primary care providers and clinics to support medical homes, developmental screening, and referrals.
• Outreach to pediatric practices in designated county serving low-income children.
• Trains providers and staff on medical home concepts, developmental screening, and referral pathways.
• Identify barriers to medical home access and develop potential solutions.
• Conducts site visits to provide technical assistance and additional training.
• Assists in design and distribution of medical home materials for providers and families.
• Collects and analyzes program data to monitor results and suggest improvements.
• Prepares project reports and procedures for senior management.
