Job Description

We offer a full-time, remote position with home office flexibility and local engagement. Candidates must live in the Ovideo/Central Florida area for daily in-person patient visits.

Join our compassionate team and help make a positive difference in an injured person’s life. As a Field Case Manager, you will work closely with treating physicians/providers, employers, customers, legal representatives, and the injured/disabled person to create and implement a treatment plan that returns the injured/disabled person back to work appropriately, ensure appropriate and cost-effective healthcare services, achievement of maximum medical recovery and return to an optimal level of work and functioning. In this role, you will:

  • Demonstrate knowledge, skills, and competency in the application of case management standards of practice.
  • Use advanced knowledge of injury types, medications, comorbidities, treatment options, and job duties to advise on treatment plans.
  • Interview disabled persons to assess recovery, including occupational or non-occupational injuries or conditions.
  • Collaborate with treating physicians/providers and utilize available resources to create and implement tailored treatment plans.
  • Work with employers and physicians to modify job duties to facilitate early return to work.
  • Evaluate and modify case goals based on patient progress and treatment effectiveness.
  • Manage workload independently, prioritize cases, and decide on effective case management strategies.
  • Complete additional duties such as attending appointments, preparing status updates, and other tasks as assigned.
Qualifications
  • Education: Associates or Bachelor’s Degree in Nursing or related field.
  • Experience: 2+ years clinical practice preferred; workers’ compensation experience a plus.
  • Skills: Effective advocacy, independent work capability, basic computer skills (Excel, Word, Outlook), strong written communication.
  • Certifications: Active RN license in good standing; URAC-recognized case management certification (CCM, CDMS, CRC, CRRN, COHN, COHN-S).
  • Travel: Reliable transportation and ability to attend in-person appointments in assigned area.
  • Internet: Reliable internet connection required.
