Job Description

This is a full-time, remote position. You will work from a home office but must be located in the Queens, NY area due to daily local travel for in-person patient appointments. Approximately 75% of travel is expected within the Brooklyn metropolitan area.

Compensation: Competitive salary plus potential monthly bonuses.

As a Field Case Manager, you will work closely with treating physicians, providers, employers, customers, legal representatives, and injured/disabled persons to create and implement treatment plans aimed at returning individuals to work and achieving maximum medical recovery. Your responsibilities include:

  • Applying case management standards of practice effectively.
  • Using knowledge of injuries, medications, comorbidities, and treatment options to advise on treatment plans.
  • Interviewing disabled persons to assess recovery status and injury attribution.
  • Collaborating with healthcare providers and utilizing resources to tailor treatment plans.
  • Working with employers and physicians to modify job duties for early return to work.
  • Evaluating and adjusting case goals based on progress and treatment effectiveness.
  • Managing workload independently, prioritizing cases, and handling related duties such as attending appointments and preparing updates.

Qualifications

  • Associates or Bachelor’s Degree in Nursing or related field.
  • 2+ years of clinical practice preferred; workers’ compensation experience a plus.
  • Ability to advocate effectively, work independently, and demonstrate proficiency in basic computer skills.
  • Bilingual in English and Spanish preferred.
  • Active RN license in good standing.
  • Reliable transportation and internet, with the ability to travel locally.

The base pay ranges from $79,000 to $105,000 annually, with eligibility for productivity-based bonuses. Total compensation depends on skills, experience, education, and performance.
