Job summary

Theoria Medical is seeking a full-time Medical Director and Attending Physician to supervise post-acute care across SNFs in Michigan. You will balance direct patient care with leadership, supported by a flexible schedule and a collaborative clinical team.

Ideal candidates hold an MD/DO, active US license, and board certification in relevant specialties. The role includes a competitive Total Rewards Package and ongoing professional development opportunities.

Qualifications

• MD or DO with valid US medical license and board certification.
• Board-eligible/new graduates encouraged to apply.
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Responsibilities

• Serve as attending physician for patients in assigned SNF facilities.
• Collaborate with NPs/PAs to manage daily patient care and provide clinical supervision.
• Deliver high-quality, patient-centered primary care including diagnosis and treatment of acute and chronic conditions.
• Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring and other quality initiatives.
