Job summary

Personalized Health Partners is recruiting a Medical Director of two West Virginia facilities to lead medical services and apply innovative technologies to improve outcomes. The role combines bedside care with strategic oversight for two centers in Bluefield and Princeton, WV.

The position requires MD/DO, WV license, ABMS board certification in relevant areas, and 5+ years in geriatric/long-term care settings; leadership of NP education is a plus.

Qualifications

• MD/DO degree required; Gerontology certification preferred.
• ABMS board certification in Internal Medicine, Family Practice, Geriatrics or Hospice/Palliative.
• 5+ years of experience with geriatric or long-term care populations; experience educating NPs.

Responsibilities

• Assume administrative authority and implement medical services, policies and procedures per regulations.
• Coordinate medical care, manage emergency procedures, liaise with attending physicians, monitor quality of care.
• Provide physician services for residents in skilled nursing facilities or clinic environments.

The Personalized Health Partners medical practice is currently recruiting a physician to serve as Medical Director of two of our West Virginia facilities: Mercer Healthcare Center located in Bluefield, WV and Glenwood Healthcare Center located in Princeton, WV. As Medical Director, you will be part of an exciting new model of care combining good old fashioned bedside manner with utilizing innovative technologies to dramatically reduce patients' return to hospital and achieve better outcomes.

Job Duties and Responsibilities

Assume the administrative authority, responsibility, and accountability of implementing our medical services, policies, and procedures according to federal, state and local regulations; and current practice of post-acute and long-term care medicine. Coordinate medical care, including emergency treatment procedures, maintain effective liaison with attending physicians, and implement methods to keep the quality of care under constant surveillance. Provide necessary physician services for those residents within skilled nursing facilities and/or clinic environment.

Qualifications

M.D. or D.O. required Gerontology certification preferred MD or DO license in State of West Virginia ABMS Board Certified in: Internal Medicine, Family Practice, Geriatrics or Hospice/ Palliative 5+ years of experience working with geriatric or long-term care population Demonstrated experience working with and educating NP’s.

Preferred Qualifications

Health care management/leadership experience Familiarity with CMS, Medicare Advantage, SNP Rules and Regulations HCC and Risk Adjusted reimbursement methodology Inpatient/SNF utilization metrics and bed day management State specific collaborative practice agreements and midlevel practitioner licensing Knowledge of EMR.
