The Village is accredited by the International Commission on Accreditation of Rehabilitation Facilities (CARF) for its Adoptions, Foster Care, Mental Health, Wraparound, and Drop-In Center services. We deliver evidenced-based behavioral health services to children, youth and adults, and has long been at the forefront of culturally sensitive foster care and adoption services.
POSITION DESCRIPTION
The Medical Coordinator is responsible for helping to assess, facilitate, and monitor all components of medical services. The Medical Coordinator will participate in weekly staffing meetings, ongoing training and stakeholders' meetings. The Medical Coordinator must adhere and apply the agency policies and procedures, code of ethics, Federal regulations, State mandates, the Flores Settlement Agreement, Trafficking Victims Protection Reauthorization Act, Perez-Olano Settlement Agreement, and the Violence against Women Reauthorization Act.
REQUIREMENTS & QUALIFICATIONS
- Minimum of 1 year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration or direct clinical care AND any of the following:
- Licensed practical or vocational nurse, (LPN/LVN) or higher health-related qualification (e.g., licensed registered nurse (RN) with a bachelor's degree), OR
- Bachelor's degree in a relevant field
- Bilingual English/Spanish, ability to read, analyzes, and interprets the most complex documents
- Computer literate in Microsoft Office applications.
- Must be able to lift up to 25 pounds on a frequent basis.
- Must perform a wide range of physical movements such as lift, carry, push, pull, bend, sit, stand, walk, climb, kneel, and crawl.
VACCINATIONS REQUIRED
- Varicella: two doses of varicella vaccine (at least four weeks apart) OR born in the United States before 1980 with a reported history of chickenpox (verified by an adult present at the time of illness) OR titers indicating varicella immunity OR documentation from a healthcare provider verifying previous infection.
- MMR: two doses of MMR vaccine (at least four weeks apart) OR titers indicating immunity to all three diseases.
- Influenza: one dose annually during flu season.
DUTIES & RESPONSIBILITIES
All of the responsibilities listed below are considered essential functions of this position. Additionally, this position is expected to ensure that the core operating values and the strategic direction of the agency are adhered to.
- Assist to schedule and comply with required medical and dental appointments and timelines.
- Assist to respond and comply to individual emergency medical needs immediately.
- Assist to obtain, compile, and maintain medical files on each admission. Ensure proper daily documentation in the Portal and maintain hard copy of required documents and medical record.
- Assist to collect, maintain, and submit weekly, monthly, quarterly, and annual reports to supervisor.
- Maintain Confidentiality guidelines meeting State regulations and the safety of client.
- Assist in submitting review and follow up all the Significant Incidents Reports regarding Medical Issues.
- Assist to schedule and provide transportation when is needed to on medical appointments.
- Assist with TAR (Treatment Authorization Request) request and follow up.
- Assist with any outstanding medical bills and insurance (Point Comfort underwriters).
- Assist in coordinating medical assessments within timeline (48 hours for medical initial examination and initial after 70 days of placement).
- Maintain a professional conduct and professional relationship with identified medical providers as well as assist in finding providers/specialist as needed.
- Attend team meetings, staffing reviews and any other training required.
- Adhere to Child Abuse Reporting requirements.
- Must be relied upon regarding task completion and follow-up meeting deadlines on time without sacrificing accuracy, work quality, or customer service satisfaction.
- Perform other duties as requested by the assigned supervisor and or management.
- Must provide child-facing services on-site.

Pay Range
$32.44/hr, non-exempt
