The Medical Coordinator leads non-clinical health service delivery by coordinating medical care for youth and collaborating with providers to ensure timely interventions.

Salary includes a $1,500 sign-on bonus and requires HIPAA compliance, strong documentation, and coordination with health professionals to ensure safety and health standards.

Qualifications

  • 1 year of public healthcare experience with care coordination, health administration, or direct clinical care.
  • LPN/LVN, RN license, or relevant bachelor’s degree.
  • Current license in the practicing state and CPR/First Aid and CPI certifications.

Responsibilities

  • Maintain HIPAA compliance and proper medical file maintenance.
  • Coordinate timely medical, dental, and related services with local providers.
  • Schedule transportation to medical appointments with program staff support.
  • Support medical providers during appointments and with medication management.
  • Educate and counsel children on disease prevention and treatment.

We are building careers that make a difference.The Medical Coordinator assumes a pivotal role within VisionQuest by spearheading the team's approach to non-clinical health service delivery. The Medical Coordinator duties encompass evaluating, facilitating, and overseeing all aspects of youth's medical care. Emphasizing disease prevention and health upkeep, the Medical Coordinator collaborates closely with medical providers to ensure children in our care receive the best health services available and timely medical intervention.

Essential Functions

  • Compliance:
  • Stay knowledgeable on all related local, State, and Federal policies and procedures regarding medical coordination and services for youth under our care.
  • Maintain Health Insurance Portability and Accountability Act (HIPAA).
  • Ensure proper maintenance of medical files on each admission.
  • Ensure proper daily documentation in VisionQuest database and specific federal agency portal and maintain hard copy of required documents and medical records.
  • Ensure all documentation including but not limited to medication administration logs meets VQ, federal agency's, and Licensing policies.
  • Medical Services Coordination & Response:
  • Ensure timely medical, dental, and other related services are provided to children with local providers within federal and state licensing timelines.
  • Schedule and provide transportation to children on medical appointments with assistance from program staff, as needed.
  • Collaborate with health professionals to ensure health and safety standards, including licensing, disease reporting, immunizations, injury prevention, and medication management for out-of-home childcare, along with awareness of community health and mental health resources for children.
  • May attend medical, dental, and mental health appointments, when applicable and needed to support the medical department or for children with complex medical needs.
  • Support the medical providers staff during appointments examination procedures when needed.
  • Ability to dispense over the counter and prescribed medication to children according to VisionQuest’s self-administration of medication policy and procedure.
  • Immediately respond and comply with individual emergency medical needs of children in care.
  • Educate and counsel children concerning their disease, treatment, and prevention of disease, as well as informing staff when appropriate.
  • Collaboration and Communication:
  • Ensure professional conduct when interacting with stakeholders, building strong partnerships.
  • Work with other members of the management team to assure successful communication and coordination between all members, leadership, and corporate personnel.
  • Attend all departmental and program meetings to ensure that up-to-date information is received and/or information of policy changes or practice are adhered to.
  • Other tasks and responsibilities as assigned by supervisors.

Education & Experience

  • Minimum of 1 year of employment in public healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care.
  • Any of the following:
  • Licensed practical or vocational nurse (LPN/LVN) or;
  • Higher health‑related qualification (Licensed Registered Nurse) or;
  • Bachelor’s degree in a relevant field.
  • License to practice must be current and accepted in the state of employment.
  • Supervisory experience. Experience as a medical coordinator in a youth shelter setting is highly preferred.

Qualifications

  • Must be 21 years of age or older.
  • Bilingual in English and Spanish is preferred.
  • Flexible schedule (evening, weekends, holidays) to facilitate program services.
  • Cleared tuberculosis test results.
  • Must be able to pass a drug test.
  • Must be able to provide proof of required vaccinations.
  • Cleared background checks from appropriate entities, including Child Abuse and Neglect and FBI criminal background.
  • Obtain and maintain CPR/First Aid and Crisis Prevention Intervention (CPI) certification.
  • Must have a valid driver’s license and be eligible to drive to facilitate program services as required.
  • Must maintain current LVN/LPN Licensure in the practicing state.

Working Conditions/Physical Requirements

  • May lift and carry up to 10 pounds frequently, up to 20 pounds occasionally, up to 50 pounds infrequently.
  • Performs tasks that may involve standing or walking 50% of the day; sitting 50% of the day.
  • May bend, stoop, reach and grasp as necessary.
  • Must be able to provide First Aid and CPR.
  • Must be able to perform physical holds as instructed in Crisis Prevention Intervention training (CPI).

The Pay Range For This Role Is 27.40 - 27.40 USD per year(888 Steven Rogers Village)
