Job summary

AdvanceCare Health Services seeks a Medical Coordinator with DDA/ECF/CLS experience to manage administrative tasks within the Medical Department. The role offers hybrid work with on-site duties once weekly and some remote work after training.

The position requires coordinating appointments, monitoring service plans, maintaining records, and liaising with families, staff, and outside providers to ensure compliant, high-quality care in Tennessee.

Qualifications

  • Experience in coordinating administrative tasks for health services or care programs.
  • Ability to prepare reports and records of individuals' program progress with accuracy.
  • Knowledge of practices and techniques for individuals with intellectual disabilities; ability to apply proper supervision and judgment.

Responsibilities

  • Function as a medical case manager for individuals served in our Family Model Program.
  • Monitor and ensure compliance with service plans per regulatory guidelines.
  • Coordinate appointments, meetings, and administrative tasks for individuals served or staff.
  • Assist individuals and their families in a supportive role.
  • Collaborate with other agencies on behalf of individuals served.
  • Assist in interviewing prospective individuals and families with the Director.
  • Maintain patient records and utilize scheduling systems.

Job description

To qualify for this position you must live within a one hour drive of Hermitage, TN.

AdvanceCare Health Services, LLC is seeking a dedicated Medical Coordinator with DDA/ECF/CLS experience to help us with our Medical Department. The ideal candidate will be responsible for managing administrative tasks and assisting in the smooth operation of the department . Working at home up to 4 days a week after initial training.

This is a hybrid role, combining work-from-home flexibility with some on-site and delivery responsibilities once weekly.

You will need to have a 27" or larger monitor, keyboard, and mouse to use with our company-issued laptop. (If you are coming to the office to pick up your equipment, we can loan you the monitors, keyboard, and mouse as well).

Applicants must have high-speed internet.

DUTIES:
  • Function as a medical case manager for the assigned individuals served in our Family Model Program
  • Monitor the individual’s service plan in accordance with the regulatory guidelines
  • Coordinate and arrange appointments, meetings, and other administrative matters on behalf of individuals served or staff members
  • Assist in a supportive role for the individual served and his/her family
  • Work with other Agencies on behalf of the assigned individual served
  • Assist Director in interviewing prospective individuals served and their families
  • Utilize computerized systems to schedule appointments and maintain patient records
  • Perform administrative duties such as filing, data entry, and office management
  • Utilize medical terminology accurately in communication and documentation
  • Work with the Director in promoting community acceptance and support through participation in community activities and/or liaison efforts with other agencies and/or entities
  • Assist in the performance of other duties as required by the needs of individuals served and as assigned by the Director
  • Perform other duties and responsibilities as assigned and as needed
QUALIFICATIONS:
  • Cooperates with anyone who evaluates the home or represents the various agencies that place individuals in the home
  • Ability to prepare necessary reports and records of individuals' program progress with neatness, accuracy, and completeness
  • Knowledge of practices and techniques of programming for individuals with intellectual disabilities Ability to exercise judgment in interpreting and implementing programs
  • Ability to implement proper practices and techniques of supervision
  • Ability to maintain effective verbal and written communication with staff, individuals' families, individuals and outside service providers
  • Must be flexible and organized
  • Strong administrative skills with attention to detail
  • Ability to multitask and prioritize tasks effectively
  • Must maintain liability insurance, good driving record, and reliable transportation
  • Must be able to work without close supervision
  • The ability to interact with a wide range of people
  • Good Computer skills: MS Word, Excel knowledge and Excel Spreadsheets, Outlook, Google Docs, and Windows.
Work Environment:

This full-time, 40-hour-per-week role offers a hybrid work arrangement, using company-provided systems to stay connected and productive. It requires a quiet, professional workspace when working from home, reliable internet, and the ability to stay focused and organized without direct supervision. Regular communication with leadership and team members is essential, and when working from home, timely responsiveness during business hours is expected.
