Job summary

The University of Louisville School of Dentistry seeks a Medical Case Manager I to support Ryan White HIV clients across Kentucky. This full-time role involves enrolment, care coordination, and data collection to ensure competent, timely dental care through the Ryan White Oral Health Program.

Responsibilities include patient enrollment, appointment attendance follow-ups, transportation logistics, and meticulous documentation in EMR and CareWare. CQI participation is also expected.

Qualifications

• Bachelor's degree required in Social Work, Sociology, Psychology, Nursing or related field or equivalent combination of education and experience.
• At least 2 years experience working within HIV care services.

Responsibilities

• Enroll new patients/clients into care for the Ryan White Oral Health Program
• Call patients to ensure they make it to their appointments
• Schedule transportation for patients when transportation to appointments is a barrier to care
• Document all client interaction and services provided in the electronic medical record and CareWare database
• Produce weekly updates of scheduled patients having obtained most recent medical labs
• Develop a comprehensive individualized care plan and monitor the plan to assess efficacy
