Job Overview – Medical Care Manager (LPN):

Compensation: $38 - $40/hour

Location: Bronx, NY

Schedule: Monday to Friday, 9:00 AM - 5:00 PM (Full-Time)

Step into a meaningful role with our client as a Medical Care Manager (LPN) in Bronx, NY. This full-time position focuses on overseeing the medical care of youth in residential and foster care settings. You will play a critical role in promoting and restoring the health of adolescents through comprehensive and compassionate nursing care.

Responsibilities as the Medical Care Manager (LPN):

• Health Assessments: Conduct home visits to assess and address the health needs of adolescents.
• Medical Coordination: Establish and maintain a medical home for adolescents while liaising with healthcare providers to ensure continuity of care.
• Client Support: Provide emotional and psychological support to clients and their families, promoting independence through education and goal setting.
• Compliance: Document and audit medical records to ensure compliance with policies and regulations.
• Medication Administration: Administer medications, monitor responses, and ensure accurate records.
• Trauma-Informed Practices: Implement and practice the Sanctuary Model tools to maintain a trauma-sensitive environment.

Qualifications for the Medical Care Manager (LPN):

• Education: Completion of an accredited practical nursing program (LPN).
• Certification: Current CPR certification required.
• Experience: Minimum of 2 years of practical nursing experience, with a strong foundation in nursing principles and care delivery.
• Skills: Strong verbal and written communication skills with a meticulous attention to detail.
• Additional Requirements: Comfortable accompanying youth to appointments and deeply committed to applying trauma-informed care principles.
