Iconic Care Management is seeking a compassionate, organized Medical Assistant to support our Principal Care Management (PCM) program. This role helps patients with complex chronic conditions receive ongoing care coordination, education, and support between provider visits.

The ideal candidate has experience in patient communication, care coordination, documentation, and working independently in a remote environment.

Qualifications

• Medical Assistant Certification is required.
• 5+ years in medical office, care coordination, or chronic care management.
• Strong computer skills and experience with EHR systems.
• Excellent verbal and written communication skills.
• Ability to work independently in a remote setting.

Responsibilities

• Support PCM program with care coordination, education and ongoing patient support.
• Communicate with patients, providers and caregivers; document interactions.
• Build and maintain relationships to ensure continuity of care.

Day-to-Day Responsibilities
Position Overview

Iconic Care Management is seeking a compassionate, organized, and self-motivated Medical Assistant to support our growing Principal Care Management (PCM) program. This position plays a vital role in helping patients with complex chronic conditions receive ongoing care coordination, education, and support between provider visits.

The ideal candidate is experienced in patient communication, care coordination, documentation, and working independently in a remote environment. This role requires excellent customer service skills, attention to detail, and the ability to build strong relationships with patients, providers, and caregivers.

This role is best suited for a self-motivated and confident Medical Assistant who thrives in an independent.

Qualifications:

• Medical Assistant Certification
• Minimum 5 years of medical office, care coordination, or chronic care management experience.
• Strong computer skills and proficiency with EHR systems.
• Excellent verbal and written communication skills.
• Ability to work independently in a remote setting.
• Strong organizational and time-management skills.
• Knowledge of HIPAA regulations and patient privacy requirements.

Pay: From $24.00/hr
