Job Description: Care Team Specialist

Location: United States, Wisconsin, Madison

Organization: Group Health Cooperative of South Central Wisconsin — BETTER TOGETHER

Employment Type: 0.90 FTE / 36 hours per week

Position Type: Hybrid (Clinic: Thursday/Friday)

Incentive: $3000 for joining GHC (prorated based on FTE)

Position Overview

The Care Team Specialist (CTS) is a vital member of the care team, supporting high-quality patient care and engagement in maintaining physical and mental well-being. Responsibilities include pre-visit preparation, monitoring patient flow, rooming patients, obtaining histories and vital signs, managing test results, supporting Epic workflows, returning calls, and assisting with examinations and procedures. CTS collaborates with RNs and providers, participating in team reviews and message management. Qualifications include completion of relevant medical training or certification.

Minimum Qualifications
  • Must meet requirements for Medical Assistant (MA), Licensed Practical Nurse (LPN), or EMT, as specified.
  • Medical Assistants: Completion of an accredited MA program and eligibility for certification within 6 months or three years of experience with current certification.
  • LPNs: Completion of an accredited LPN program and current licensure in Wisconsin or within 6 months of employment.
  • EMTs: Completion of approved training and valid Wisconsin license (EMT, AEMT, or Paramedic).
  • CPR/AED certification required within four months of employment.
  • Ability to travel and access to personal transportation; valid Wisconsin driver’s license preferred.
  • One year of relevant clinical experience is desirable.
  • Knowledge of clinical procedures, equipment, computer applications (experience with EPIC preferred), medical terminology, pharmacology, and administrative procedures.
  • Strong communication, interpersonal, and confidentiality skills; ability to work with diverse populations.
  • Physical ability to perform tasks such as lifting and standing for prolonged periods.
