Access Healthcare Associates in San Jose, CA is seeking a Field Medical Assistant to provide compassionate, field-based care to seniors, with a Monday–Friday schedule. The role focuses on direct patient care, assessments, and coordination with the healthcare team.

What You’ll Do:

As a Field Medical Assistant, you’ll play a vital role in driving exceptional patient care and ensuring seamless coordination across our healthcare team—providing oversight, guidance, and exceptional care to a dedicated caseload of patients. You’ll play a vital role in ensuring seamless communication, coordination, and support across providers, patients, and families.
