Job summary

CompuGroup Medical is seeking a patient-focused Care Coordinator to support Medicare RPM and CCM participants. This on-site role engages high-risk patients, guiding them through care plans via structured outreach and monitoring, with a Monday–Friday schedule and no evenings or weekends.

Responsibilities include outbound outreach, documentation in EHR, device management, and patient education to improve adherence and outcomes.

Qualifications

• Active National Certified Medical Assistant required.
• Minimum 2 years healthcare experience.
• Minimum 1 year in Chronic Care Management services.
• Strong communication and interpersonal skills.
• Comfortable with high-volume outbound calling.
• Strong documentation and EHR proficiency.
• Willingness to work on-site Monday–Friday.

• Accurately document all call outcomes utilizing multiple client EHR and other systems.
• Conduct monthly check-in calls for RPM and CCM patients.
• Review and monitor synced vital signs from RPM devices.

• Schedule and order RPM devices, educate patients on usage, and confirm contact details.
• Conduct structured health assessments and follow personalized care plans.

We are seeking a motivated and patient-focused Care Coordinator to support patients enrolled in Medicare’s Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.

This is not a traditional back-office Medical Assistant role. Instead, you will serve as a key point of contact for high-risk patients with multiple chronic conditions — helping them stay engaged, compliant, and on track with their care plans through structured outreach and monitoring. This on-site role offers a stable Monday through Friday schedule with no evening or weekend commitments.

Your Contribution:

• Make 60+ outbound calls per day to eligible patients using our dialer system
• Accurately document all call outcomes utilizing multiple client EHR and other systems.
• Conduct monthly check‑in calls for RPM and CCM patients
• Review and monitor synced vital signs from RPM devices
• Explain RPM program benefits in clear, patient‑friendly language
• Schedule and order RPM devices, educate patients on proper usage, and confirm patient contact details.
• Conduct structured health assessments and follow personalized care plans tailored to each patient
• Provide education on nutrition, exercise, medication adherence, and lifestyle choices
• Obtain and document verbal enrollment consent per program requirements
• Track and document patient progress accurately and consistently

Your Qualification:

• Active National Certified Medical Assistant Required (To expedite the hiring process, please upload NCMA/NRMA credentials)
• Minimum 2 years of healthcare‑related experience
• Minimum 1 year in related Chronic Care Management services
• Strong communication and interpersonal skills
• Comfortable with high-volume outbound calling
• Strong documentation and EHR proficiency
• Willingness to work on‑site during regular business hours, Monday‑Friday
