Job summary

Graceful Aging Home Health Care seeks a Medical Assistant to provide clinical and administrative support to nurses, therapists, and leadership. You will help organize patient records, track orders, and assist with scheduling and coordination of home health visits.

The role emphasizes documentation accuracy, HIPAA compliance, and collaboration with an interdisciplinary team to ensure efficient care delivery.

Qualifications

• High school diploma or equivalent.
• Completion of a Medical Assistant training program.
• Medical Assistant certification or registration preferred.
• Minimum of one year of experience in a medical office or healthcare setting preferred.
• Current CPR certification preferred.

Responsibilities

• Provide administrative and clinical support to the Director of Nursing, Clinical Manager, nurses, therapists, and other team members.
• Organize and maintain patient clinical records; review charts for completeness.
• Track physician orders, plans of care, certifications, recertifications, discharge documents, and other clinical paperwork.
• Fax or securely transmit orders to physicians and providers; document receipt as required by policy.
• Coordinate home health visits, referrals, labs, diagnostics, and equipment; assist with appointment scheduling.
• Notify licensed clinicians of changes in patient conditions or concerns; assist with discharge and transfer coordination.
• Maintain confidentiality and comply with HIPAA; support quality audits and compliance activities.

Education

Medical Assistant training program

CMA/RMA/CCMA credential (preferred)
