Job summary

Partners in Care Foundation is seeking a Care Navigator to support Kaiser Permanente Medi-Cal members and dually eligible individuals. The role emphasizes high-touch engagement in the Emergency Department, connecting members to ECM, TCS, CCM, Social Work, MFA, and other Kaiser resources.

The navigator will collaborate with ED leadership and multidisciplinary teams to improve care coordination, reduce avoidable ED utilization, and ensure smooth transitions after discharge.

Qualifications

• Experience with Medi-Cal/Medicare Dual Eligible or CalAIM programs.
• Familiarity with hospital or Emergency Department operations.
• Strong communication and patient engagement skills.
• Ability to navigate complex systems and coordinate across multidisciplinary teams.
• Experience with care coordination or social service linkage preferred.
• Able to adapt to fast-paced pilot workflows.

Responsibilities

• Identify eligible members from real-time ED census lists and outreach.
• Engage with members in person within the Emergency Department.
• Coordinate enrollment into Enhanced Care Management (ECM).
• Provide navigation for ECM or other care coordination programs.
• Connect members to Kaiser services (TCS, CCM, Social Work, MFA).
• Document outreach and interventions per pilot workflows.
• Follow up with members after ED discharge.
• Participate in ED huddles and staff workflows.
