Qualifications

• Current RN license in the State of California.
• BLS certification required.
• ACLS required within 3 months of hire/transfer.
• Chemo Certification required within one year of hire/transfer.
• Stroke Education required annually for those working at QVH.

Responsibilities

• Provides care coordination for a group of patients for a designated time frame.
• Coordinates and implements patient care specific to patient age group; ensures quality care is delivered safely.
• Assesses biophysical, psychosocial, environmental, self-care and educational needs to develop and revise the plan of care.
