Responsibilities

• Gather data, interpret findings, and tailor care plans to age-related needs.
• Document care and patient progress toward outcomes.
• Direct and supervise others in delegated nursing activities.
• Teach patients and families about the plan of care.
• Prepare patients for discharge and manage time/resources to deliver care.
• Administer medications and provide direct patient care.
