Qualifications

• Performs plan of care interventions, including medication administration and clinical treatments.
• Plans and documents patient care provided.
• Monitors and communicates patient condition to care team and family.
• Assists patients with daily living activities and mobility.
• Coordinates care with other disciplines for transfers, discharges, and referrals.
• Assesses and educates patient/family based on age and culture.
