Responsibilities

• Manages individualized, outcome oriented nursing care through the nursing process.
• Develops, discusses, and updates a prioritized plan of care for each patient and documents outcomes.
• Initiates admission assessment within 8 hours and educates patient/family as needed.
• Ensures safety, documentation, and regulatory compliance within departmental policies.
• Facilitates discharge planning and communication of care goals by 12:00pm.
