Qualifications

• BLS certification required within 30 days of hire.

• Develop and evaluate patient-centered care plans to ensure high-quality outcomes.
• Document patient care journeys with precision and ensure clear communication across the care team.
• Assess and facilitate discharge planning from start of care for smooth transition to post-acute settings.
