Responsibilities

• Assist nursing staff in delivering basic patient care and maintain a safe, organized environment.
• Record vital signs and patient data, reporting findings to RN/LPN/LVN.
• Support patients with meals, bathing, grooming, and mobility.
• Assist with positioning, ambulation, and use of mobility aids.
• Collect urine and stool samples and monitor blood glucose as directed.
• Communicate patient information effectively to care team and maintain confidentiality.
• Maintain infection control practices and proper PPE usage.
• Participate in performance improvement initiatives and risk management.
• May be required to observe patients continuously as needed.
