Responsibilities

• Record vital signs, intake/output, and report findings to RN/LPN/LVN.
• Assist with meals, bathing, grooming, linen changes, and mobility.
• Position patients and assist with ambulation using mobility aids.
• Collect urine/stool samples and monitor glucose levels as directed.
• Communicate patient information effectively to the care team.
