• Recent experience in acute care, physician's office, home health or long-term care.
• Preference for the setting you are applying to.
• Ability to communicate effectively with staff and patients.

Responsibilities

• Provide basic patient care including mobility and comfort assistance.
• Monitor health and vital signs and report changes.
• Assist with feeding, grooming and personal care needs.
• Serve as liaison between patient and nurse for care updates.
• Document accurately and timely for patient care in the EHR.
