Job summary

York General is seeking a compassionate Licensed Practical Nurse (LPN) to join the Med/Surg team for night shifts in a 25-bed critical access hospital. You will deliver high-quality, patient-centered care and collaborate with a dedicated team to meet patient needs.

Graduate of an accredited nursing program with current BLS, ACLS, and NRP certifications is required. Strong communication, empathy, and attention to detail are essential for success in this role.

Responsibilities

• Assess and plan patient care using the nursing process; implement orders and therapies.
• Provide direct patient care including medications, IV therapy, hygiene, nutrition, and positioning.
• Prioritize and delegate care; supervise delegated tasks where appropriate.
• Assess education needs and discharge planning; provide teaching and support to patients and families.
• Communicate with physicians and health care team; participate in rounds and document patient progress.

What You'll Do

• Utilizes the nursing process in assessing and evaluating patient needs and problems, planning and implementing nursing care, evaluating patient response and progress toward outcomes and revising the plan of care accordingly.
• Provides direct patient care and implements physician orders (administers medications, treatments, IV therapy; meets needs for hygiene, nutrition, positioning and activity; applies other interventions appropriate to patient and SO needs).
• Prioritizes, organizes, delegates care activities as appropriate to patient and shift needs; provides supervision of delegated tasks.
• Assesses readiness and need for patient/SO education including discharge planning needs; provides appropriate teaching and support.
• Communicates and collaborates with the physician and other health care providers to meet patient needs; participates in rounds, utilizes lab results, progress notes and other parts of the medical record to keep informed regarding patient condition.
• Documents assessments, evaluations, and interventions; provides other verbal, written and automated communication needed.
