Job summary

Senior Resource Group is seeking Med Aides to assist with resident medication delivery and daily living activities in Assisted Living and Memory Care settings. You will observe, document, and report resident conditions, and coordinate with Healthcare Providers and families as needed.

The role emphasizes accuracy in medication handling, adherence to eMAR, and timely communication with care teams to ensure resident safety and well-being.

Qualifications

• Arizona caregiver certification required.
• Ability to assist with medications and monitoring.
• Excellent observation and documentation skills.

Responsibilities

• Prepare and assist with medications as prescribed.
• Document medications given in eMAR.
• Communicate changes in medication orders to residents, providers, family, and nurse.
• Observe and document changes in resident conditions in progress notes.
• Document and follow up on resident concerns in shift reports.
• Re-order medications and ensure proper labeling and storage.
• Notify nurse of medication discrepancies.
• Provide first aid and respond to emergencies.
• Enter data into Yardi EHR and maintain logs.
• Document monthly weights and vitals in Yardi EHR.
• Maintain a clean medication room and carts.
• Complete narcotic counts with oncoming shift.

Job Description

In partnership with the Director of Assisted Living and Memory Care Director, Med Aides are responsible for assisting with resident medication delivery and management, providing activities of daily living, and observing, documenting, and reporting resident conditions.

Essential Duties & Responsibilities

• Preparing and assisting with medications as prescribed by Healthcare Providers to residents in Assisted Living and Memory Care.
• Documenting medications given and/or not given in eMAR.
• Communicate to the Resident, Healthcare Providers, Family/POA, and Director/Nurse regarding change in medication orders, refill challenges, or missed scheduled doses of medication.
• Observing, reporting, and documenting changes in resident conditions in Yardi EHR progress notes and Nurse to Med Aide Communication Log.
• Documenting and follow up on resident concerns on the End of Shift Report.
• Medication re-ordering and receiving, so residents do not run out of medication and confirming medications are logged, properly labeled, and stored.
• Notifying Director/Nurse of any medication discrepancies.
• Providing first aid when needed and responding to medical emergencies.
• Assisting with completing internal incident reports and data entry into Yardi EHR.
• Documenting alert charting, once per shift, in Yardi EHR progress notes.
• Documenting monthly weights and vital signs as needed in Yardi EHR.
• Maintaining a clean and organized medication room, med carts, and medication overflow areas.
• Completing a narcotic count with oncoming shift.
