Job summary

SYNERGY HomeCare of Conroe is looking for compassionate, dependable caregivers to provide personalized in-home support based on each client’s needs.

You will assist with daily living activities, mobility, meals, medication reminders, light housekeeping, transportation, and meaningful companionship while protecting client dignity and privacy.

Responsibilities

• Provide transportation or accompany clients to appointments as needed.
• Offer companionship and social interaction to reduce isolation.
• Encourage independence and engagement of clients.
• Observe, report changes in client condition or needs.
• Follow individualized care plans and company policies.
• Communicate professionally with clients, families, and the office.

What You’ll Do

As a caregiver, you will provide personalized in-home support based on each client’s individual needs. Responsibilities may include:

• Assisting with activities of daily living, including bathing, dressing, grooming, toileting, and personal hygiene

• Providing mobility assistance, transfers, and fall-prevention support

• Preparing meals and assisting with feeding when needed

• Providing medication reminders according to the client’s care plan

• Assisting with light housekeeping and laundry

• Providing transportation or assistance with errands and appointments, when applicable

• Offering meaningful companionship and social interaction

• Encouraging clients to remain as independent and engaged as possible

• Observing and reporting changes in a client’s condition, behavior, or needs

• Following individualized care plans and company policies

• Communicating professionally with clients, families, and the SYNERGY HomeCare office

• Providing dependable, compassionate care while protecting each client’s dignity and privacy
