Job summary

Provide CIC is seeking an MDT Coordinator to support Integrated Care Teams in providing non-clinical support to Clinical Managers, Community Matrons and District Nursing Teams. The role involves coordinating a multi-disciplinary group of professionals to deliver proactive care for frail and complex patients, and to manage tasks, caseloads and equipment within SystmOne.

The role also covers managing ambulatory catheter clinics, coordinating referrals, waiting lists, and booking spaces.

Qualifications

  • Experience coordinating multidisciplinary teams in community health or social care.
  • Strong caseload management and task-tracking skills.
  • Familiarity with SystmOne patient records is advantageous.

Job description

We are looking for a Multi-Disciplinary Team (MDT) Coordinator to work as part of the Integrated Care Team, providing non -clinical support to the teams Clinical Manager, Community Matrons and District Nursing Teams. They will also coordinate a team of professionals from multiple disciplines in the community, such as social care, mental health and GP's, to provide a proactive approach to care, specifically those with complex needs and increasing frailty.

The key duties of an MDT Coordinator is to provide day to day support for the Community Nursing Teams and the Matrons through the use of accurate record keeping via our Patient Electronic Records on SystmOne. This includes handling incoming tasks to the team, caseload management, equipment ordering and support with using the SystmOne software. The MDT Coordinator is responsible for managing a caseload of patients identified as frail or complex and will create a Multi-Disciplinary Team surrounding this cohort of patients to identify their needs and implement care plans through collaborative working with community services including the Community Nursing Teams, GP's, Social Workers, Mental Health Teams, Hospice Nurses, Safeguarding Leads and Paramedics. The MDT Coordinator will act as the point of contact for health and social care professionals to access the Frailty Pathway and is expected to have a good understanding of local services and support networks available in our community. The MDT Coordinator will also manage Ambulatory Catheter Clinics facilitated by our Community Nursing Teams, this will include accepting new referrals, managing waiting lists, and booking clinic space and appointments.
