Job summary

CareOne at Brookline seeks an experienced MDS/Clinical Reimbursement Coordinator to manage the resident assessment process and care planning for long-term care residents. The role involves completing MDS/CAA, participating in care plan meetings, and ensuring compliance with federal and state regulations.

You will work with an interdisciplinary team, develop individualized care plans, and contribute to high-quality patient outcomes.

Qualifications

• RN licensure in the state of practice with PPS and OBRA assessment experience.
• Experience in a hospital or long-term care facility or related setting.
• Knowledge of long-term care laws, regulations, and guidelines.
• Proficient with Windows; Accu Med experience preferred.
• Experience completing MDS and Care Plans requested.
• 1-3 years MDS experience or LTC RN experience eligible for training.
• Ability to work independently and adapt to month-end close.
• Detail oriented with accurate, timely assessments.

Responsibilities

• Direct the Resident Assessment Process and assist with MDS/CAA completion.
• Develop individualized Care Plans and identify resident needs and outcomes.
• Execute MDS/RAI care plan functions.
• Complete MDS items, CAAs and Care Plans as designated by the facility.

The MDS/Clinical ReimbursementCoordinator will be responsible for, but not limited to:

1. Direct the Resident Assessment Process through assisting with the completion of the Minimum Data Set (MDS) and CAA's.
2. Participate in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes as required by Federal and State regulations.
3. MDS/RAI Care Plan Functions
4. Complete nursing assigned MDS items, CAA's and Care Plans as designated by the facility.
5. Participate in the resident Care Plan Meetings.

Position Requirements:

• RN licensure in the state of practice and experience in PPS and OBRA assessments
• Previous experience in a hospital, long-term facility, or other healthcare related facility.
• Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care.
• Computer skills-Windows applications, computer experience Accu Med experience preferred.
• Previous experience in completing MDS and Care Plans is required.
• 1-3 years of MDS experience or RN with LTC experience that can be trained in assessment role
• Can work independently
• Flexible with schedule for month end close
• Detail oriented and able to accurately and timely complete assessments.
