MDS Coordinator Summary

As the MDS Coordinator, you will serve as the RAI/MDS Coordinator for our center. Your responsibilities will include reviewing and processing MDS assessments in accordance with CMS and LTCI regulations and guidelines. Must be an RN.

MDS Coordinator Responsibilities

• Collaborate with nursing caregivers and other disciplines to ensure accurate care plan goals.
• Effective communication with various disciplines to facilitate timely completion and transmission of RAI/MDS.
• Prepare accurate reports on Quality Measure/Quality Indicators (QM/QI) from RAI/MDS data.
