Century Care Management Inc in Elkin, NC seeks an experienced MDS Coordinator to lead the MDS team and ensure timely, accurate documentation in accordance with state requirements. You will coordinate the MDS/PPS program, develop and monitor care plans, and work with the interdisciplinary team to assess resident needs.

Qualifications include NC RN license, MDS and Case Mix experience, and familiarity with EHR systems (PointClickCare preferred).

We're looking for a new team member to lead our MDS Team who desires to make a difference in the long-term care arena and to positively touch lives. The Minimum Data Set (MDS) Coordinator is responsible for timely and accurate completion, coordination, recording, updating, and monitoring of all Minimum Data Set documentation required by state and other appropriate authorities.

Specific responsibilities include:

• Regulates and maintains the MDS/PPS program and completion schedule
• Develops and coordinates appropriate nursing care plan from MDS/RUG category
• Evaluates effectiveness of nursing care plans, and updates MDS/RAI process within completion guidelines
• Works in collaboration with the Interdisciplinary Team to assess the needs of the resident
• Monitors MDS and care plan documentation for all residents. Ensures documentation is present in the medical record to support MDS coding.

• Experience with Electronic Health Records, PointClickCare Preferred
