Job summary

Northwell Health in New York seeks a skilled RN to coordinate MDS assessments and ensure CMS/Medicaid compliance. This role maintains MDS records and guides the nursing care process from assessment to evaluation. You will ensure timely completion of MDS for OBRA/PPS schedules and support reimbursement efforts.

The position requires a BSN within 5 years, NY RN license, and 1–3 years of relevant experience. Strong regulatory knowledge and independent judgment are essential.

Qualifications

• Experience coordinating MDS processes for CMS/OBRA/PPS requirements.
• Strong knowledge of regulatory standards and documentation practices.
• Ability to collaborate with multiple disciplines to ensure compliant resident care plans.

Responsibilities

• Coordinate and ensure completion of interdepartmental MDS for new, readmitted, quarterly and annual reviews.
• Complete documentation for Medicare reimbursement.
• Review care plans for regulatory compliance and quality improvement.
• Report deficiencies in assessments to drive immediate improvement.
• Complete Resident Assessment Protocols (RAPs) for residents.
• Participate in care plan meetings and staff education on MDS documentation.
• May chair the Comprehensive Care Plan (CCP) meetings and support performance improvement initiatives.
