Job summary

AristaCare Health Services is seeking a MDS Registered Nurse Coordinator to ensure development and completion of the Resident Assessment Instrument (MDS) in compliance with state/federal requirements. This full-time on-site role collaborates with the interdisciplinary team to support quality of care and patient-centered goals.

The candidate will coordinate RAI processes, participate in resident care planning, and help implement programs to maximize reimbursement while maintaining high care

Qualifications

• Current active RN licensure in New Jersey (RN NJ) required.
• Experience as an MDS Coordinator required.
• Experience with MDS completion and submission processes.
• Solid knowledge of Medicare/Medicaid regulations and benefit guidelines.

Responsibilities

• Conduct patient assessments to determine health status and care level.
• Ensure accurate and timely RAI completion.
• Coordinate interdisciplinary participation in RAI and resident care plans.
• Assist in developing programs to maximize reimbursement potential and improve quality of care.

Job description

MDS Registered Nurse Coordinator - (Full time: On-site, Not remote) This position is responsible for ensuring the development and completion of the Resident Assessment Instrument through the completion of the (MDS) in accordance with state/federal requirements and the policies/goals of the facility. This person is a key participant of the interdisciplinary team, assisting in ensuring quality of care, continuity of services, and individualized patient-centered plans and goals.

Duties and Responsibilities

• Conducts patient assessments and determines the health status, level of care, and any subsequent changes.
• Ensures accurate and timely RAI completion.
• Coordinates interdisciplinary participation in completion of the RAI and resident care plan.
• Assists in developing and implementing programs and procedures to maximize the reimbursement potential of the facility and improve quality of care.

Requirements

• Current active RN NJ licensure
• MDS Coordinator experience
• Has experience in MDS Completion
• Knowledge of Medicare/Medicaid regulations and benefit guidelines
