Job summary

Durham-Hensley Health and Rehabilitation, Inc. seeks an RN or LPN with LTC experience to lead the RAI coordination team. You will manage MDS timelines, ensure accurate data entry, and collaborate with physicians, nurses, and rehab staff to support Medicare residents.

The role requires strong knowledge of RAI processes, compliance with federal guidelines, and a compassionate approach to elder care. Nursing leadership is essential.

Qualifications

• Active RN or LPN license from a Nurse Licensure Compact state.
• Certified Nursing Assessment Coordinator or eligible.
• Prefer 1 year of Long-Term Care experience.
• Experience coordinating and completing the RAI process with accuracy.
• Ability to work with elders and maintain professional demeanor.

Responsibilities

• Assure resident safety at all times.
• Chair a team and demonstrate leadership and supervisory skills.
• Accurately record daily documentation in electronic systems.
• Complete and submit required forms per regulations and policy.
• Maintain confidentiality of resident information and rights.
• Ensure MDS and RUG data are timely and accurate for billing.
• Coordinate with management on hospital documents for RUG scoring.
• Attend Medicare-related meetings and implement committee recommendations.
• Train nursing staff to document ADL functioning and changes.
