Job summary

Simpsonville Post Acute in Simpsonville, SC is seeking an experienced MDS Nurse (RN) to coordinate and oversee resident assessments, ensuring timely MDS completion and PDPM optimization. You will collaborate with the interdisciplinary team to achieve positive resident outcomes.

The ideal candidate is an RN with MDS knowledge and strong organizational and communication skills, proficient in Microsoft Office and PCC.

Qualifications

• Current, unrestricted RN license.
• Graduate of an accredited School of Nursing.
• Knowledge of MDS assessment process and regulatory requirements.
• Strong organizational, communication, and documentation skills.
• Proficiency with Microsoft Office.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments. (Medicare PPS, Quarterly, Annual, Significant Change, OBRA)
• Coordinate the interdisciplinary MDS process and ensure regulatory compliance.
• Lead weekly PDPM meetings for skilled residents and monitor PDPM performance.
• Analyze QM data and provide recommendations for improvement.
• Assess new admissions and communicate resident level of care to the team.
• Maintain assessment schedules and regulatory timelines.
• Submit MDS assessments electronically and perform corrections as needed.
• Schedule and conduct resident care conferences; develop and review care plans.
• Collaborate with clinical staff to update care plans and outcomes.
• Provide training to MDS staff and new coordinators; ensure HIPAA compliance.
