Job summary

Elevate Care Windsor Park is seeking a dedicated MDS Nurse to join our interdisciplinary team. You will coordinate and complete MDS assessments and ensure OBRA/PPS compliance, documenting thoroughly to support quality care.

In this role, you’ll collaborate with nursing staff, physicians, and other disciplines, review care plans, and educate staff on MDS processes and regulatory standards. Illinois RN or LPN license required; experience in MDS coordination preferred.

Qualifications

• Current RN or LPN license in Illinois.
• Experience in MDS coordination in skilled nursing or LTC setting preferred.
• Knowledge of RAI process, MDS 3.0, and Medicare guidelines.
• Strong attention to detail and organizational skills.
• Ability to work collaboratively with interdisciplinary teams.
• Proficiency with electronic health record (EHR) systems.

Responsibilities

• Coordinate and complete the MDS assessments in accordance with federal and state regulations.
• Ensure accurate and timely completion of OBRA and PPS assessments.
• Collaborate with nursing staff, interdisciplinary teams, and physicians to gather data for assessments.
• Review resident care plans and make recommendations based on assessment findings.
• Participate in QAPI initiatives.
• Educate and support staff regarding MDS processes and documentation standards.
• Monitor changes in regulations and ensure ongoing compliance.
