Job summary

Laurel Health Care Company is hiring a Care Management Nurse, MDS Nurse to manage the RAI process and coordinate care plans for residents. You’ll perform MDSs, CAA’s, and PPS-related tasks within regulatory timeframes and collaborate with physicians, nursing staff, and therapy teams.

A strong understanding of RAI and PPS is strongly preferred, with an RN license required. The role emphasizes autonomy and clinical involvement beyond bedside care, with the opportunity to join a national provider

Qualifications

• RN with knowledge of the RAI/ MDS process and PPS basics.
• Experience as an MDS nurse or in a similar assessment role.
• Ability to coordinate care plans and communicate with multidisciplinary teams.

Responsibilities

• Completes MDS, CAA’s and care plans within regulated timeframes.
• Assesses residents through physical exams, interviews and chart review.
• Coordinates with physicians, nursing, social services, therapy and dietary staff.
• Reviews hospital and outside information for assessments and care planning.
• Maintains PPS board and monitors Medicare days and RUGs utilization.
• Keeps current with AANAC requirements.

The Care Management Nurse, MDS Nurse works the RAI process and conducts assessments and care plan coordination for those residents assigned. Some responsibilities of the MDS nurse include:

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS’
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications:

• Registered Nurse, RN
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
• Experience as an RN MDS Nurse
