Job summary

Laurel Health Care Company is seeking an experienced MDS Nurse for our Defiance facility to manage RAI assessments and care plan coordination. This role emphasizes autonomy and direct involvement in patient care beyond bedside nursing, with a focus on accurate documentation and timely regulatory timelines.

You will collaborate with physicians, nursing, social services, therapy, dietary and activity staff, ensuring comprehensive resident care and compliance with PPS requirements.

Qualifications

• Registered Nurse (RN) licensed to practice in the state of Ohio.
• Experience with the MDS process and resident assessments.
• Knowledge of RAI and PPS concepts is strongly preferred.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulatory time frames.
• Assesses residents through physical exams, interviews and chart reviews.
• Coordinates care needs with physicians, nursing, social services and therapy staff.
• Reviews hospital data and external information for accurate assessments and plans.
• Coordinates, identifies and initiates significant changes to MDS; prepares PPS meetings and monitors PPS board.
• Keeps current with AANAC requirements.

Responsibilities:

The Care Management Nurse, MDS Nurse works the RAI process and conducts assessments and care plan coordination for those residents assigned. Some responsibilities of the MDS nurse include:

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS’
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications:

• Registered Nurse, RN
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
• Experience as an MDS Nurse
