Job summary

Sun City Post Acute is seeking an MDS Nurse (LVN/LPN) to coordinate and complete MDS assessments in compliance with federal and state regulations. You will document resident care accurately, support reimbursement, and collaborate with interdisciplinary teams to promote quality outcomes.

The role requires current LVN/LPN license, familiarity with MDS 3.0/RAI, and proficiency in EHR systems, with strong organizational and communication skills to engage residents and families in care planning.

Qualifications

Current LVN/LPN license in the state of practice.
Knowledge of MDS 3.0, RAI process, and Medicare/Medicaid reimbursement guidelines.
Proficiency in electronic health records (EHR) systems.
Strong organizational skills with attention to detail and accuracy.
Ability to work collaboratively with interdisciplinary teams.

Responsibilities

Complete MDS assessments and CAAs within required timelines.
Ensure accuracy of resident clinical documentation to support care planning and reimbursement.
Maintain compliance with CMS, state, and facility guidelines.
Collaborate with interdisciplinary team members to develop individualized care plans.
Monitor resident progress and update care plans as needed.
Communicate with residents and families regarding care planning.
