Sandridge Post Acute is seeking a dedicated MDS Nurse (LVN/LPN) in Mesa, AZ to coordinate and complete MDS assessments per federal and state requirements, ensuring accurate documentation and supporting reimbursement.

You will collaborate with interdisciplinary teams to develop care plans, monitor progress, and participate in care conferences and QA initiatives; strong EHR skills and knowledge of MDS 3.0 are essential.

Qualifications

Current LVN/LPN license in the state of practice.
Experience in long-term care or skilled nursing preferred.
Knowledge of MDS 3.0 and RAI processes.
Familiarity with Medicare/Medicaid reimbursement guidelines.
Strong organizational skills and attention to detail.
Proficient with electronic health records (EHR) systems.

Responsibilities

Complete MDS assessments and CAAs within required timelines.
Ensure accuracy of resident documentation to support care planning and reimbursement.
Maintain CMS/state/facility regulatory compliance.
Collaborate with interdisciplinary teams to develop individualized care plans.
Monitor resident progress and update care plans as needed.
Participate in care conferences and quality improvement initiatives.
Ensure MDS submissions meet Medicare/Medicaid requirements.
Assist with audits and surveys by providing accurate documentation.
Stay current with regulatory changes impacting MDS requirements.
Coordinate with nursing, therapy, social services, and dietary teams.
Provide staff education on documentation standards.
Communicate effectively with residents and families about care planning.
