Hillsboro Post Acute seeks a qualified MDS Nurse (LVN/LPN) to coordinate and complete MDS assessments in compliance with CMS and state regulations. You will document resident care accurately and collaborate with interdisciplinary teams to support quality outcomes.

Responsibilities include timely MDS/CAAs, ensuring regulatory accuracy for Medicare/Medicaid, educating staff on documentation standards, and participating in care conferences and audits.

Qualifications

• Must have LVN/LPN license in the practicing state.
• Knowledge of MDS 3.0 and RAI processes.
• Proficiency with electronic health records (EHR).
• Strong organizational skills and attention to detail.

Responsabilités

• Complete MDS assessments and CAAs within required timelines.
• Ensure accuracy of resident documentation to support care planning and reimbursement.
