Job summary

Cambridge Healthcare Services, LLC is seeking an MDS Nurse/LVN to deliver direct patient care and support the MDS Coordinator in assessing health needs. You will help identify health issues, set goals within care plans, and coordinate interventions across the facility.

The role involves charting, collaboration with IDT, and ensuring compliance with MDS processes and federal/state standards. Strong communication and PC familiarity are essential.

Qualifications

• Must have LVN license in good standing.
• CPR and BLS certification required.
• Minimum 1 year of skilled nursing experience preferred.
• Must be able to speak, read, write English.
• Proficiency with PCC is a plus.
• Computer or tablet literacy to enter resident data.

Responsibilities

• Provide direct care to residents per the patient care plan.
• Assess care planning process completion and monitor changes.
• Complete medical charting accurately and timely.
• Participate in patient-centered quality improvement initiatives.
• Perform Resident Assessment and develop care plans after interviews.
