Job summary

Murrietahcc is seeking an MDS Nurse with LVN licensure to provide direct care to residents and assist the MDS Coordinator with assessments and care planning in a California facility.

Responsibilities include preparing and coordinating Resident Assessments, maintaining medical records, and supporting quality improvement efforts with the interdisciplinary team. LVN licensure, CPR, and BLS are required.

Qualifications

• Education / Licensure: LVN training from an accredited school or college.
• Valid LVN license in good standing.
• Valid CPR and BLS card.

Responsibilities

• Provide direct care to residents per patient care plan and assist with problem identification and medical interventions.
• Assess the resident care planning process and monitor changes in care levels.
• Complete charting in medical records accurately and timely.
• Implement patient-centered quality improvement initiatives to improve the quality of care residents receive.
• Perform Resident Assessment including collecting medical history, medications, and functional abilities to develop an accurate resident care plan.
