Qualifications

• Graduate of an accredited School of Nursing.
• Current Ohio LPN or RN license.
• Minimum of 1 year of MDS nursing experience.
• 3+ years in long-term care preferred.
• Proficiency in Microsoft Office and EMR (PointClickCare).

Responsibilities

• Conduct initial and ongoing MDS assessments per regulations.
• Complete MDS sections, RAIs, and care planning documentation.
• Collaborate with interdisciplinary team for accurate resident needs.
• Educate staff on MDS requirements and regulatory standards.
• Ensure compliance with long-term care regulations.
• Maintain up-to-date MDS records and timely data transmission.
